I understand I'll be required to fill out a liability waiver and release form before my child can participate in their first session. I understand that basketball involves certain risks of injury and I hereby assume those risks. *

I understand I'll be required to sign a media release form in person. I grant permission for my child to be photographed or videotaped while participating in Austin Wildcats activities, and for those images to be used for promotional purposes. *

I have read and agree to the Austin Wildcats Code of Conduct and CARE rules. *
